IVFD UK’s equality watchdog insists on transgender fertility services
The UK’s equality watchdog has demanded that the country’s National Health Service offer scarce fertility services to transitioning transgender people. At the moment, 208 clinical commissioning groups around the UK decide whether to provide the “gamete extraction and storage” services to patients, but the Equality and Human Rights Commission (EHRC) claims that many will not provide these to transgender patients.

In this uncomplicated paper, the authors have used a new Microfluidic sorting [ZyMot ICSI Sperm Separation Device] to prepare a sample of highly motile sperm from a residual neat semen sample and then tested this sample using sperm chromatin dispersion assay [SCDA]. They reported that the sample had almost 100% sperm with normal DNA integrity. Compared to swimup and Density Gradient preparations, the microfluidic device was better.|This should not be a surprise given the well established link between both motility and morphology to DNA Fragmentation. What was impressive was that the sample was almost 100% normal sperm. Clearly the device selects for only the sperm with the strongest forward progressive motility which coming directly from neat semen will still be uncapacitated and with no whiplash type motility. One would need to consider whether the cost (not known but it will not be cheap) outways the advantage. It would only be suitable for samples with reasonable concentration and as the authors noted, there was just enough to perform ICSI.|The reason why it may be important is two fold. The first is that sperm with elevated DNA Fragmentation has been linked to poor pregnancy outcomes and increased miscarriage. Clients, whose husbands have elevated DNA fragmentation and who miscarry, may well feel poorly treated (in a legal sense) if other sperm preparation techniques that were available and not used. Secondly, ICSI these days is used at a very high rate and individual embryologists remain in control of sperm selection and there is no assurance they will all select the same sperm. Using a microfluidic sorting device in some ways takes control of which sperm are being injected since they will all be of normal DNA quality.| It will be interesting to see future articles using this device to see if the observations are repeated and applied to IVF cases to confirm better obstetric outcomes.|
