IVFD Preimplantation genetic testing for aneuploidy by microarray analysis of polar bodies in advanced maternal age: a randomized clinical trial
WIDER IMPLICATIONS OF THE FINDINGS: The observation that the similarity in birth rates was achieved with fewer transfers, less cryopreservation and fewer miscarriages points to a clinical benefit of PGT-A, and this form of embryo selection may, therefore, be considered to minimize the number of interventions while producing comparable outcomes. Whether these benefits outweigh drawbacks such as the cost for the patient, the higher workload for the

IVFD Children born through IVF may have higher risk of hypertension. By age 16, premature vascular aging persists in apparently healthy adolescents
Children conceived through assisted reproductive technologies may be at an increased risk of developing arterial hypertension early in life, among other cardiovascular complications, according to a Swiss study published today in the Journal of the American College of Cardiology

IVFD The effect of female body mass index on in vitro fertilization cycle outcomes: a multi-center analysis
Conclusions: A BMI above the normal range was an independent negative prognostic factor for multiple outcomes, including cycle cancellation, oocyte and embryo counts, and OCP. These negative outcomes were most profound in women with class-II/III obesity, ovulatory dysfunction, or PCOS.

Title
Does polar body biopsy for chromosomal abnormalities improve the pregnancy rate
Summary
The results of an ESHRE trial looking at the efficacy of Polar body biopsy and genetic testing for aneuploidy (PGT-A) has found that selecting oocytes that did not have a meiotic aneuploidy produced t
Comment
Many recognise that chromosomal abnormalities (CA) in IVF embryos was a significant reason for disappointing implantation rates and pregnancy failures. CA can arise either during the initial meiotic cell division that produces the polar body or later during cleavage. It was hypothesized that testing the polar body was a less invasive process that left the embryo intact whereas testing the embryo may cause major stress that resulted form removal of 1 or 2 blastomeres.| Therefore, it seemed logical to sample the polar body. However, its removal was complicated especially performing ICSI afterwards. ESHRE organised a clinical trial to explore this hypothesis. The results were complicated by the limited number of clients who volunteered for the trial. The results suggested that there was no significant gain in the implantation rate nor a decrease in the miscarriage rate. This the trial showed that there was little gain in performing PGT-A with the exception that fewer treatment cycles were needed to achieve a pregnancy. Whether the cost is outweighed by fewer cycles remains to be seen.| Currently trophectoderm biopsy at the blastocyst stage is the most popular form of genetic testing in IVF mainly because of the easy of biopsy, the larger number of cells obtained and the knowledge that the embryo itself has not been damaged – only the trophectoderm.| However there was some valuable information in this report about the incidence of CA in the sampled population. These statistics include 36% of cases had ZERO normal oocytes ( i.e. one third of cases did not have any normal oocytes) and 65% of all oocytes were aneuploidy (i.e. genetically abnormal). It would seem that the logical question to arise from this study must surely be why is the aneuploidy rate so high!|
