IVFD Optimal endometrial thickness to maximize live births and minimize pregnancy losses: Analysis of 25,767 fresh embryo transfers
Conclusions: Endometrial thickness is strongly associated with pregnancy losses and live births in IVF, and the optimal endometrial thickness threshold of 10 mm or more maximized live births and minimized pregnancy losses.

The results showed that t3 (< 14h) was the most relevant parameter related to live birth gender (OR 2.452, 95% CI 1.071–5.612, P = 0.03). WIDER IMPLICATIONS: Although the use of eFET has steadily increased in recent years, a significant increase in LBR with eFET was solely noted in hyper-responders and in patients undergoing PGT-A. The group from Reproductive Medical Center at Tongji College of Medicine looked at over 1700 cycles and identified from fresh and frozen transfers cases where all the embryos transferred resulted in a live birth and where the gender of the child was confirmed in followup and time-lapse data existed. They looked at the time for each embryo to pass through each cleavage phase and related these times to the gender of the children. For SET, this is straight forward and for DET twin pregnancies they had the data for each of the embryos transferred but not for each individual embryo. For the purposes of this paper, this was not an issue.| They reported that male embryos reached many of the cleavage steps in a shorter time than female embryos (an observations seen in many animal species but rarely in humans) but the time the embryo passed through the 3 cell stage (a very brief stage between the 2-cell stage and the 4 cell stage) was significantly faster in male embryos. In their discussion, they reviewed the observations of two other studies that examined a link between time lapse based morphokinetic parameters and embryo gender and who did not see a difference. Interestingly, both of these studies used different culture medium to that described in this paper suggesting that each media may have varying morphokinetic criteria and looking for a subtle gender difference may be less apparent in some media than in others. There have been reports of media and morphokinetic criteria variations so this argument has some strength.|Assuming these observations will be repeated and confirmed, their report opens up the possibility of gender selection based on morphokinetic criteria rather than any other genetic testing. This may proceed without any legal oversight or documentation. Given the tight limits of this technique, there may be considerable error and any clinic considering using cleavage time rates to anticipate gender and allow clients to choose which embryos to transfer had better ensure their legal disclaimers are watertight. Nevertheless, if the data is consistently confirmed with this culture media (Vitrolife), then clinics need to be aware of this and have their P&P in place to deal with any enquiries - especially from couples who are not infertile!!.| They also made the point that if gender differences exist (in some media) then the time constraints used by many algorithms made be bimodal and lead to loss of sensitivity in using time-lapse to select the optimal embryo form transfer.|
