Distribution: 5740-5760 Posted 27/5/2014 Embryo Ranking Challenge-Invitation
Comment:Mainly from RBMonline this week. Some great articles inc a free access on the predictive value of time-lapse modelling and another article on phospholipase C-zeta . Plus links to articles on cumulative pregnancy rates where FET are included in the outcome. This is a key shift in how one calculates efficacy of IVF. Plus more on the business of IVF. Bit of a worry! Two good patient stories as well.. Note, FertAid has changed its registration process now requires access via a single portal. Survey-2014 Laboratory Census

IVFD Human PLCζ:: exhibits superior fertilization potency over mouse PLCζ:: in triggering the Ca2+ oscillations required for mammalian oocyte activ
A sperm-specific phospholipase C-zeta (PLCζ::) is believed to play an essential role in oocyte activation during mammalian fertilization. Sperm PLCζ:: has been shown to trigger a prolonged series of repetitive Ca2+ transients or oscillations in oocytes that precede activation. This remarkable intracellular Ca2+ signalling phenomenon is a distinctive characteristic observed during in vitro fertilization by sperm

IVFD Heterochromatin variants in human karyotypes: a possible association with reproductive failure
The heterochromatin variants occurred more frequently in the reproductive failure group than in the controls (P = 0.006). Regarding individual chromosomes, chromosome 9 variants were most frequently found in the study and control groups, and they also occurred more frequently in the reproductive failure group. The most significant difference between the study and control groups was found for the 9qh+ variant (P = 0.026), and although the results

IVFD Chromosome analysis in embryos from young patients with previous parity
According to the results, informative in 152 couples, the proportion of euploid embryos was significantly lower in 53 nonpregnant women when compared with 99 women with term pregnancy (49% versus 75% respectively, P < 0.001). In addition, in 21 nonpregnant patients, all embryos transferred were found to be chromosomally abnormal. These findings confirmed the limited prediction power of embryo morphology on implantation but also the relevance of c

IVFD Impact of high serum progesterone during the late follicular phase on IVF outcome
The aim of this narrative, nonsystematic review is to address the nonconsensual issues of the reasons and consequences of premature progesterone elevation during ovarian stimulation. The determination of the optimal threshold above which progesterone elevation may have an impact on IVF outcome is still a matter of debate because it is likely related to the patient's profile. Additionally, it is still uncertain whether the duration of progesterone

IVFD On the cost and prevention of iatrogenic multiple pregnancies
The aim of this study was to determine whether, using real data from a major regional insurance carrier for the interval 2005–2009 covering obstetric costs requiring hospitalization and neonatal costs through the first year, it was possible to show that the cost saved by eliminating iatrogenic multiple births would be adequate to fund a protocol to minimize iatrogenic multiple births. The net savings on an annual basis for the study group of 13,

IVFD Quality of additional embryos transferred on pregnancy outcomes in IVF: predictions using a mathematical approach
Transfer of additional embryos, irrespective of embryo quality, always increased pregnancy rate and MPR. When implantation rate was low, there was a marked increase in pregnancy rate but only a relatively small increase in MPR. In contrast, with higher implantation rates, the increase in pregnancy rate was mainly due to the increase in MPR, with the same singleton pregnancy rate. Transfer of additional embryos, irrespective of embryo quality, fol

IVFD Is there a critical endometrioma size associated with reduced ovarian responsiveness in assisted reproduction techniques?
Significantly lower numbers of follicles ≥:16 :mm (P = 0.024) and oocytes retrieved (P = 0.001) in the ovaries containing endometrioma were observed. In patients with endometriomas ≥:30 :mm, endometrioma size was the most influential contributor to the total number of follicles and oocytes retrieved. Ovarian endometriomas result in reduced response to ovarian stimulation, compared with the response of the contralateral normal

IVFD Coasting, embryo development and outcomes of blastocyst transfer: a case–control study
The duration of coasting up to 4 :days did not affect progression to blastocyst stage. The multivariate model showed that coasting (OR 1.73, P := :0.004) and the number of oocytes retrieved (OR 1.17, P := :0.001) were positively correlated with blastocyst formation. Coasting, a measure to reduce the risk of OHSS, does not impair blastocyst development or clinical outcome. Coasting should remain an effective measure to pre

IVFD Freeze-all is a superior therapy to another fresh cycle in patients with prior fresh blastocyst implantation failure
Multiple logistic regression indicated freeze-all was associated with greater cumulative live birth rate than was a fresh cycle (OR 1.9, 95% CI 1.1–3.3, P = 0.0287). These findings suggest that, following implantation failure with fresh blastocysts, patients have a significantly greater chance of live birth with freeze-all and subsequent thaw than with another fresh cycle.

IVFD Limitations of a time-lapse blastocyst prediction model: a large multicentre outcome analysis [Open Access]
- but the model's results did not match observations in the test cohort because it rejected a large proportion of embryos from the test cohort that actually resulted in pregnancy. This hypothetical experiment highlights the limitations of predicting blastulation only. Crucially, it illustrates that both sensitivity and specificity are important parameters when developing embryo selection models for prospective clinical use.

Rights of sperm donors-clinic obligations
Summary
The link contains a news story about sperm donors seeking to have access to children conceived from their sperm donation.
Comment
In most countries, the act of sperm donation is covered by laws that define the sperm donor as no legal rights to children conceived from its use. However, where the donor is know to the recipient, there may be some areas where access rights may be questioned. Does the clinic have a role in ensuring that donors and recipients seek written legal advice before the insemination. One wonders if a clinic may be exposed to legal consequences if they fail to ensure that all parties have a binding legal contact and not just rely on the law that is designed for anonymous donation?

Development of models for use of time-lapse embryos for transfer
Summary
Kirstine Kirkegaard et. al article in RBMonline (see link) reports on a multicentre study using cleavage rates (C1,C2,C3) to identify embryos for transfer
Comment
The study used a model developed by Conaghan et al.(2013) to predict blastocyst development using cleavage time cutoff. The study showed that embryos choosen under this model had a high implantation rate (22%). This was fine expect that the embryos that were rejected as cleaving outside the model set points also had an implantation rate of 14%. The authors discussed a model predicting blastulation may not be the same as implantation and the model, if rigorously applied, may lead to a significant number of forgone pregnancies. Maybe the newer models may include set point to day 5 and improve predictability but there does suggest concerns if the model was applied without due diligence. Comments?
