IVFD Influence of interpregnancy interval on birth timing
While short IPI is a known risk factor for preterm birth, our data show that inadequate birth spacing is associated with decreased gestational age for all births. Pregnancies following short IPIs have a higher frequency of birth at all weeks of gestation prior to 39 and fewer births ≥::40 weeks, resulting in overall shortened pregnancy duration.

Women who have short intervals between pregnancies of less than 18 months are more likely to see a decrease in the length of subsequent pregnancies, finds a new study. A note from the SMH details the movement of both private IVF Companies into the International market.
Comment
The establishment of IVF Companies onto the stock-market may be good in that it generates money for new development and research. More than one player should lead to more efficient service and competitive prices which is good for IVF clients. It may also lead to prices as high as the market could bear and more difficult work practices (eg more ICSI or day 5 transfers and fewer freezings). One concern though is that in Australia, the IVF industry is largely underwritten by Medicare and one thing is for certain, Medicare will not continue to generously fund shareholder funder companies as they did when it was a cottage industry. Look at pathology if you care to look into the future. When funding is limited, the smaller clinics will the ones to suffer. Anyone care to comment on this.

This study from an Italian group suggests daughters from hyperandrogenic PCOS clients have an elevated risk from autism and related disorders.
Comment
More recently, another study (see IVFDaily)that elevated testosterone in amniotic fluid is also related to autism. More recently, a review in Hum Reprod failed to find an association between IVF and autism. One wonders though whether the risk to a few PCOS clients is being lost within the IVF population at large. Another study from Sweden also found a very minor link between IVF and autism disorders where their focus was on ICSI and poor sperm quality. These studies looking at an endocrinology origin for some autism links should cause some clinics to discuss inclusion in their risk statements to clients about autism.

Background monitoring work to support the efficacy of time-lapse embryo culture.
Comment
Probably difficult to perform this with human embryos but it remains an important background observation for those clinics using time-lapse culture systems. Many of the algorithms developed using timing of cleavage rates need to be viewed against the impact of red-light compared to repeated exposure to room light wavelengths and microscopes commonly used for embryo assessment.

The study asks the question whether time-lapse monitoring is able to identify the viability of shrunken blastocysts after vitrification and thawing.
Comment
Interesting article that addresses one of the issues of blastocyst vitrification- that of the viability of embryos whose cavity has collapsed after warming. They reported only 7% of transfers involved shrunken blastocysts and that their pregnancy rate was still an impressive 40% (given the age of the women was unclear. These embryos exhibited reduced oxygen uptake but a proportion resumed expansion and hatching after 5 hours of extended culture. Data does suggest that more (at least 40%) will be able to expand and hatch (since pregnancies followed). Unsure whether time-lapse added any more information that what may be gained from visual checking. Still it give some confidence that if the shrunken blastocyst as not re-expanded by the time of transfer, the prognosis of pregnancy remain very high.

Summary
IPI- interpregnancy interval- is linked to the likelihood of have a pre-term pregnancy. An IPI of less than 12 months is associated with a 53% chance of <39 term pregnancy.
Comment
Apparently, a short IPI is a known risk factor for preterm birth. This large study confirms these observations. This was more pronounced for negro populations compared to non-negro populations. Unlike fertile populations, IVF populations conceptions are more measured and couples returning for a second pregnancy can be counselled to delay treatment if their IPI is too short. This may be more effective with frozen embryos where maternal age is less critical. It is well established that delaying treatment for women more than 35 years may reduce their chance of conception. However, if a pregnancy has arising from a fresh transfer, couples may well be advised to defer a FET treatment if the IPI is less than 12 months. It would be interesting to know the standard IPI for IVF couples and asking whether the incidence of preterm pregnancies may be linked to a short IPI.

Nitric oxide (NO) is reported to play significant a role in oocyte activation and maturation, implantation, and early embryonic development. Lower NO levels with age suggest increased oxidative stress and the decrease in antioxidant machinery. Previous work, which showed that supplementation with NO delays the oocyte aging process by diminution of oocyte ooplasmic microtubule dynamics (OMD), cortical granule (CG) loss, ZP dissolution timing and improves spindle and chromosomal integrity. While this work was conducted on mice and requires specialised probes, the relationship to ovarian aging and decreased fertilisation rates and embryo viability reminds IVF staff that there may be other reasons for ovarian aging and in mice at least, NO supplementation with NO (mechanism unclear) in part could reverse NO effects on ovarian aging. Interesting concept to treating older women!
