Physical activity before IVF and ICSI cycles in infertile obese women: an observational cohort study
Our results demonstrate that the chances to obtain a pregnancy and a baby are 3-fold higher in obese infertile patients who does physical activity regularly in comparison with those who does not, suggesting that regular physical activity before ART cycles improves the reproductive performance in obese women irrespective to body weight loss.

With an ever-ageing female patient population, egg donation is an increasingly common treatment in infertility. ESHRE's own annual reports on fertility treatments in Europe show a rise in egg donation cycles from 15,028 in 2007 to 24,517 in 2010 (to 4.05% of all treatments). Given the concerns about female (and male) obesity on their offsprings health, this article suggests that clinics treating obese females could justify a need to enter an exercise programme.
Comment
There is a growing body of evidence to suggest the weight of the female will impact on their sons future health especially their weight. While this study does not address weight per sea, it is interesting that exercise can have such a dramatic effect on a couples chance of a live birth (7% to 24%). Possibly of more importance is the real problem of weight on pregnancy. A clinic that actively limits patients by weight may expect to have a higher pregnancy rate than a clinic that does not limit by weight. Clinics may also benefit on asking obese clients of their exercise level and point out the advantages on delaying treatment and entering an exercise programe.

Comment
Many national rules have arbitrarily imposed an upper age on donors based upon quite mild rises in miscarriage and infertility. However, in this study where the live birth rate (LBR) was partitioned, donor age was independent of outcome. Indeed, the younger donors (<20 years)behaved poorest (the same with younger women). Many of the rules are based upon generalised genetic risk assessments and are conservative in nature. This study suggests that pregnancy rates are very similar between men below and above 40 years of age. With newer assessment tools such as DNA fragmentation tests, samples from men over 40 years of age with normal semen profiles and DNA fragmentation may well give be suitable donors. The only caveat is that couples know the age of the donor before accepting them and the risk for this is in the signed consent form.

A study from Denmark exploring the fertility experience of the parents of children conceived between 1969 and 2006 and whether the children were hospitalised for a psychiatric disorder.
Comment
5% of all children came from women with a fertility problem and overall these children were 33% more likely to have been hospitalised for a psychiatric disorder. The disorders included schizophrenia and psychoses, affective disorders, anxiety and other neurotic disorders, mental and behavioural syndromes including eating disorders, mental retardation, mental development disorders including autism spectrum disorders, and behavioural and emotional disorders including attention deficit hyperactivity disorder (ADHD). Wow!! even thought the relative risk was small, the study is nevertheless worrisome. The speakers(presented at ESHRE) point out the study cannot discriminate between the clients and the treatment but suggest the former is a more likely source. One wonders if they are related to male infertility, age or some other socio-economic grouping. When this topic is published as it surely will, clinics will need to consider whether to screen at risk clients, include this in the list of risks or just ignore it for the present.
