Climate change is associated with male:female ratios of fetal deaths and newborn infants in Japan
Conclusion(s): The recent temperature fluctuations in Japan seem to be linked to a lower male:female sex ratio of newborn infants, partly via increased male fetal deaths. Male concepti seem to be especially vulnerable to external stress factors, including climate changes.
The Testicular and Epididymal Expression Profile of PLCζ: in Mouse and Human Does Not Support Its Role as a Sperm-Borne Oocyte Activating Factor
In summary, we show for the first time that PLCζ: is compartmentalized as part of the acrosome early in human and mouse spermiogenesis and is secreted during sperm maturation in the epididymis. Most importantly, no evidence was found that PLCζ: is incorporated into the detergent-resistant perinuclear theca fraction where SOAF resides.
Sperm content of postacrosomal WW binding protein is related to fertilization outcomes in patients undergoing assisted reproductive technology
Our clinical data from infertile couples demonstrate significant correlations between sperm PAWP levels and both fertilization rates and normal embryonic development after ICSI. Considering its proposed role in the initiation of oocyte activation, we suggest that PAWP could have potential applications in the diagnosis and treatment of infertility.
Women's experiences of endometriosis: a systematic review and synthesis of qualitative research
Significant evidence gaps were identified: there was inadequate investigation of women's experiences of endometriosis-associated infertility and of the impact of reduced social participation on perceived support and emotional well-being, and limited or no inclusion of the experiences of adolescent and post-menopausal women, women from low socioeconomic backgrounds, women who do not identify as Caucasian, and non-heterosexual women.

The appropriate population for testosterone replacement therapy is men with hypogonadism, experts say. The treatment has vast potential for adverse cardiovascular outcomes associated with its use in inappropriate populations, they add. Though testosterone use has sharply increased among older men in the past decade, the experts say that testosterone therapy should be limited to men who meet the diagnostic guidelines for hypogonadism.

Summary
A number of women are at greater risk of mental health issues after unsuccessful IVF than others. But who and what is the role of an IVF clinic in managing this.

What responsibility does each IVF clinic have in assisting couple who fail to conceive after fertility Treatment. This interesting and important paper by Gameiro, et.al in Hum.Reprod provides some answers as to who may be at the greater risk on mental illness over continued need to be a parent. Couples with male related or unexplained infertility as are older women are at lesser risk than those younger women with primary infertility (no children) and female related conditions. Knowing this, surely clinics with a good counselling program may consider making contact with such couples every few years with advice, support areas or counselling. One problem is that most couples continue to move and will be lost to the clinic and another may be that maintaining contact stops the couple from moving on. Perhaps knowing those clients at greater risk may provide a clinic with more focused exit tools and programmes than before.

An interesting article looking at the risks of autism in ART conceived children used DNA copy number and mutation data from reference and ART chidlren. They found no difference in the incidence of autism related DNA in ART children compared to reference information. If such analysis does indeed translate to childhood outcomes then this study provides a prima facie case that IVF does not increase the risk of autism. It means not having to wait a number of years and complete a difficult study programme to prove there is no increased risk for autism following ART treatment. It does not mean such studies do not need to be done but IVF clinics may proceed without real concern about it.

A great paper that looks at temperature (??caused by climate change) and the rate of miscarriage and found that male fetuses appeared to be more susceptible to temperature stress leading to higher sex ratios among miscarriages and a appropriate decrease in sex ratios (more females) live births. There is considerable animal data (reptiles) suggesting that the sex ratio were temperature dependent but this data suggests that it may also apply to some degree in humans. Not only will it influence data collections systems but this pales into insignificance to couples conceiving a male fetus. I suspect this data will be difficult to confirm but when so may lead to the use of PDG to undertake sex selection to promote a female conception. Of final interest is why!

While many clinics use progesterone to maintain endometrial receptivity, others have both in the past and now use hCG to maintain luteal function to supply progesterone. One aspect of using hCG is that it also may mimic embryo secretions. This study has explored this feature and failed to identify any beneficial effect of uterine exposure to hCG. The data suggests that embryo derived hCG appears to function only to as a stimulant of of corpus luteum

Obsese male have impaired acrosome reaction (AR) profiles.
Comment
Testing for the acrosome reaction used to be a popular sperm function test but these days it is rarely discussed yet it remains the critical step for all IVF inseminations. This study reminds all that both in vivo and in vitro, this critical step needs to be completed after sperm zona binding. It appears that in obese males, more sperm undergo premature AR rendering a smaller population available to the oocyte and of these, fewer are likely to respond appropriately. It is a big step between in vitro testing to what happens naturally but it is not unreasonable to argue that in vitro IVF inseminations using sperm from obese males may need a review of concentrations added to the oocyte to compensate for spontaneous acrosome loss?? Good to see work still being done in this sometimes forgotten but critical step.

An important publication directed towards clinicians who are responsible for fertility preservation in young boys/men undergoing cancer treatment.
Comment
Semen collection and cryopreservation for fertility preservation is an emotive and sometimes difficult process for young men recently diagnosed with cancer and about to undergo chemotherapy. Often marriage and children are their furthest thought, some may not even have become sexually active. This study identifies that projected dose is related to the future likelihood of adult sterility and therefore allows clinicians and clinic staff to gauge the extent of sample number for cryopreservation. The study also identifies that about 25% of clients are likely to be sterile as a result of treatment. The remaining appear to be either normal or oligospermic, both conditions can easily be treated with IVF or ICSI.

In the latest FertAid 2014 Survey, staffing levels were the key area of concern, suggesting that the historical clinical staff loading of 125-150 collections/staff member/per year may no longer of applicable. Laboratory directors need firm guidelines to convince clinic management (who are increasingly administrative rather than scientific) that the clinic is at risk due to inadequate laboratory staff loading. Recent changes in technology including time-lapse imagery, blastocyst culture and PGS impact significantly on the time pressures on staff who still need to complete task withing the biological time-frames of gametes and embryos. It can be assumed any legal action against errors will take a dim view on inadequate staffing levels.
