Comment:Limited distribution this week. Thought the article on follicle volumes and the article on poorer cardiometabolic profiles in childhood were both of interest. Also the value of day 5 ET for women over 40 in the cumulative pregnancy rate worth further studies.

We make the point that morphological markers alone cannot predict the success of the early embryo, which depends on the correct orchestration of a myriad of physiological and biochemical activation events that progress independently of the maternal or zygotic genome. Since previous attempts to identify metabolic markers for embryo quality have failed and there is no evidence that the intrinsic nature of gametes and embryos can be improved in the
Efficacy and safety of late-start Corifollitropin-alfa administration for controlled ovarian hyperstimulation in IVF: a cohort, case–control study
Conclusions: CFα: may be started on day 4 of the cycle obtaining results comparable to those of a COS using day 4-start daily rFSH, with significantly less injections and a similar risk of OHSS.
The value of automated follicle volume measurements in IVF/ICSI
Conclusion: This overview of previously published literature shows how SonoAVC offers advantages for clinical practice, without losing any accuracy or reliability. Doctors should be motivated to the general use of follicular volumes instead of follicular diameters.
Comparison of hCG triggering versus hCG in combination with a GnRH agonist: a prospective randomized controlled trial
There was no statistically significant difference in the number of COCs or pregnancy rates. However, the number of patients who received at least one embryo of excellent quality was significantly higher (p = 0.001) in the group with the combined triggering (45 out of 61 patients or 73.8%) versus the group with hCG triggering alone (28 out of 59 patients or 47.5%). The
number of cryopreserved embryos was also higher in this group.
Cumulative live birth rate and assisted reproduction: impact of
female age and transfer day [PDF]
Conclusion: In women less than 35 years, excellent CLBR can be achieved irrespective of the transfer day. For women 40 years and above, better results of CLBR are observed with Day 5 transfers. Our findings may impact the counseling of couples considering IVF treatment.
Herpes Gestationis after Ovodonation: Is Placenta the only Target of the Immune Reaction?
We present a case of HG occurring in a surrogate mother whose pregnancy was achieved via egg donation and in vitro fertilisation (IVF). The disease appeared at 26 weeks and 6 days of gestation and subsequently worsened despite the administration of oral and topic corticosteroids. A healthy baby was delivered at 35 weeks and 2 days of gestation by elective cesarean divtion and the clinical manifestations resolved in the postpartum period.
The dynamic changes of anti-Mullerian hormone and inhibin B during controlled ovarian hyperstimulation in decreased ovarian reserve women and the effe
Conclusion: Serum AMH and INHB levels were not directly related to clinical pregnancy, dynamic serum AMH and IHNB levels were positively correlated with COH outcomes.
The effects of glucuronic acid and N-acetyl-D-glucosamine on in vitro fertilisation of porcine oocytes
Supplementation with 0.005 mM glucuronic acid plus 0.005 mM GlcNAc during oocyte maturation produced significantly higher rates (P < 0.05) of cleavage and blastocyst formation by 48 and 144 h after IVF compared with all other groups. These results indicate that supplementing with 0.005 mM glucuronic acid and 0.005 mM GlcNAc during oocyte maturation decreases the incidence of polyspermic penetration by increasing perivitelline space thickness and
Estimating the net effect of progesterone elevation on the day of hCG on live birth rates after IVF: a cohort analysis of 3296 IVF cycles
Live birth rates were not significantly different between cycles with and those without PE when a bivariate analysis was performed [odds ratio (OR): 0.78, 95% confidence interval (CI): 0.56–1.09]. However, when a multivariable analysis was performed, controlling for the effect of the aforementioned confounders, live birth rates (OR: 0.68, 95% CI: 0.48–0.97) were significantly decreased in the group with PE on the day of hCG.
Subfertility and assisted reproduction techniques are associated with poorer cardiometabolic profiles in childhood
At the age of 5–6 years, fasting glucose levels were higher among children conceived through ovulation induction and IVF–ICSI compared with those of children of fertile couples (adjusted difference: ovulation induction: 0.4 :mmol/l, 95% CI 0.2 to 0.6: IVF–ICSI: 0.2 :mmol/l, 95% CI 0.0 to 0.5). Blood pressure was higher in children of subfertile couples compared with that of children of fertile couples (adjusted difference systolic blood
Using cumulative pregnancy rates (CPR-Fresh + FET) shows day 3 x Day 5 same for under 35 but better for women >40
Comment
Using cumulative pregnancy rates where the chance of pregnancy is calculated by collection rather than transfer allows one to look at the whole chance of pregnancy. In this study, the CPR is the same over 3 cycles for day 3 and day 5 transfers for women <35 years of age. However for women>40 (who have blastocysts i.e. good embryos), day 5 transfer out perform day 3 transfers. The reasons for this is unclear but has been seen before and may well be true. It may well be one of embryo selection and patient selection based on embryo number and quality.
