IVFD Risk of prostate cancer for men fathering through assisted reproduction: nationwide population based register study
Conclusions: Men who achieved fatherhood through assisted reproduction techniques, particularly through ICSI, are at increased risk for early onset prostate cancer and thus constitute a risk group in which testing and careful long term follow-up for prostate cancer may be beneficial.

Conclusion(s) PGT-A did not improve overall pregnancy outcomes in all women, as analyzed per embryo transfer or per ITT. There was a significant increase in OPR per embryo transfer with the use of PGT-A in the subgroup of women aged 35–40 years who had two or more embryos that could be biopsied, but this was not significant when analyzed by ITT.

IVFD Embryonic aneuploidy rates are equivalent in natural cycles and gonadotropin-stimulated cycles
Result(s): Aneuploidy rates were equivalent in unstimulated and stimulated IVF cycles. The prevalence of aneuploidy in natural cycles increased with the age of the female partner in a manner identical to that seen in stimulated IVF cycles. Finally, sustained implantation rates of euploid blastocysts were equivalent in natural and stimulated IVF cycles.

WIDER IMPLICATIONS OF THE FINDINGS: Undergoing ART treatment without the presence of endometriosis was not associated with an increased risk of ovarian cancer, which is reassuring. Whether ART treatment increases the risk of ovarian cancer among women with endometriosis needs further investigation.

Are aneuploidy rates in natural and stimulated cycles different?
Summary
In this USA Study, aneuploidy rates in natural and stimulated cycles were the same.
Comment
Aneuploidy (the presence of an abnormal number of chromosomes) is thought to be the main cause of unsuccessful implantation and of miscarriage. Many IVF cycles routinely test for aneuploidy in usable blastocysts to try and reduce the risk of failed pregnancy and increase the time to pregnancy by only transferring euploid embryo. While there remain some arguments over whether the testing of aneuploidy reduces the implantation potential, the high rates of aneuploidy is a powerful reason why it is offered to IVF clients using IVF. The question of why there are high rates in IVF is difficult to answer until this study from a private clinic in the USA. |Published in Fertility and Sterility, the authors tested the incidence and the implantation rate by age group in 369 women attempting IVF in a natural, unstimulated cycles. From 329 oocytes from 307 cases proceeding to oocyte recovery, 302 mature oocytes were recovered (92%), 275 fertilised and 147 developed to blastocysts. Following PGT-A, 83 (57%) were euploid !| Impressively, 79 proceeded to transfer leading to 56 pregnancies (71%) and 48 (61%) viable pregnancies with a fetal heart at 8 weeks. Compare this the clinics age matched date for stimulated cycles of 80% pregnancy rate ( +ve hCG, n=1657) and 63% viable pregnancy rate. |It is well recognized that the aneuploidy rate is a function of age, increasing from 25% in women less than 35 years of age rising about 80% form women over 40 years of age. When the authors compared the aneuplody rate for women in either natural or stimulated cycles and reported the rates were virtually identical. In other words, the high rates of chromosomal errors found in older women in stimulated cycles were also observed in women in a relative natural ovulatory cycle and that ovarian stimulation did not influence this rate. This is an important observation that answers critics of IVF that the stimulation is the main cause of the high aneuploidy rates overall and especially in older women. |In the discussion the authors reviewed past publications noting that early reports suggested a relationship because the rate was linked to higher doses of gonadotrophins. Later publications suggested otherwise when the rates were dissected by age. As is in this report, age is the key factor in whether the chromosomes separate correctly and meiosis (and later and both in men and women). It also thrusts very clearly, the implication of delaying conception. |The authors also noted that for logistical reasons, ovulation was induced by both hCG and a GnRH agonist so there remains a tiny caveat on these observations that ovarian stimulation does not increase the risk of aneuploidy. What was impressive was the viable pregnancy rate (the authors called this the sustained implantation rate or SIR) in natural IVF cycles. Even though there was some attrition in the number of women proceeding to transfer, for those who did achieve a frozen embryo transfer, the chance of pregnancy was as good (almost) as those who requested stimulation. Given the issue associated with surplus embryo storage and the cost in some countries of gonadotrophins, maybe more clinic could consider offering natural cycles to more clients. |
