IVFD Preimplantation genetic testing for aneuploidy: a comparison of live birth rates in patients with recurrent pregnancy loss due to embryonic aneuploidy
A large portion of pregnancy losses in the RPL group might be due to aneuploidy, since PGT-A reduced the overall incidence of pregnancy loss in these patients. Although PGT-A did not improve the live birth rate per patient, it did have the advantage of reducing the number of embryo transfers required to achieve a similar number live births compared with those not undergoing PGT-A.

Does MACS post processing improve the quality of a sperm preparation?
Summary
At first glance the answer is no but there is more to the story.

In this 2016 article from a Turkish clinic, the quality of the sperm suspension after either swim-up or density gradient centrifugation (DGC) preparation followed by an additional isolation only MACS positive cells was investigated using a number of interesting tools. MACS (magnetic-activated cell sorting ) collects sperm that have lipid membrane associated with normal and non-apoptotic cells using annexin V-labelled beads that have a high affinity for externalized phosphatidylserine molecules. |They compared the sperm that bound the MACS beads to the feeder sperm population prepared by either swim-up of DGC and asked the question whether the populations were different. Does MACS post treatment result in a sperm population that appears better. |What was interesting about this paper was the tools they used to investigate to sample. They applied TUNEL (for DNA fragmentation), Chromomycin A3 (CMA3) staining for sperm chromatin integrity, PLCz for oocyte activation potential after fertilisation and for oocyte fertilization (Izumo-1, sperm-oocyte fusion protein). |They observed that the number of sperm, motile sperm and morphologically normal sperm were less after MACS screening. They did report that the porportion of TUNEL-negative (sperm with little DNA fragmentation) was increased but in this study to at a significant level. Of interest was that the proportion of PLCz and Izumo-1 did not change. They concluded that given the time and cost, MACS post processing did not add any additional value to the routine swim-ip for DGC procedures. |The first observation that can be made is that swim-up or DGC do extract from semen, a sperm population that is remarkably good in terms of PLCz and Izumo-1 qualities. The second observation was that even in this study, statistically significance was not achieved (maybe in a bigger sample it may do so) in selecting sperm using apoptotic-free markers. But it did improve at a P<0.1 level the number of sperm with healthy DNA. Given the association between miscarriage and DNA fragmentation, this alone may warrant its use.| Finally, it has to be said that just because the MACS processing did not produce a significantly better, more uniformily healthy sperm suspension, does not mean the technique should be discared. The real question is it harmful. Clearly it is not. Therefore applying MACS to a sperm prepation means that the sperm selected for ICSI must on all probability be more likely to be better. Since the MACS processing appears to lead to a loss of sperm numbers, this is of no consequence for ICSI as the authors pointed out. There is an arguement to be made to try to select the best sperm for injection and this article suggests that MACS post processing may go some way towards this goal. |Finally, the authors did report that there were fewer morphologically normal sperm after MACS processing. They did not speculate on this but it is difficult to understand how this can be. One would have expected an increase. Does this mean that some sperm with normal morphology do have an external membrane linked to apoptosis. It would be interesting to see further work on this maybe redefining the definition of normal sperm morphology.|
