In a cohort of present-day Britons, these carriers have more siblings, fewer miscarriages, and less bleeding during early pregnancy suggesting that the Neandertal progesterone receptor alleles promote fertility. This may explain their high frequency in modern human populations.

IVFD Is Ovarian Response Associated With Adverse Perinatal Outcomes in GnRH Antagonist IVF/ICSI Cycles?
Conclusions: No association was found between ovarian response and adverse perinatal outcomes in antagonist IVF and intracytoplasmic sperm injection cycles. Future, larger scale and prospectively designed investigations are needed to validate these results.

Does an ancient form of the progesterone receptor have a role in fertility?
Summary
A recent study from Germany has looked at an ancient Neandertal variant form of the progesterone receptor (NPR) and reported some observations that its presence may impact on a woman’s fertility.
Comment
In a fascinating, open sourced article from Germany, the authors have examined the role of an ancient form of androgen receptor in European populations and found that when the DNA databank and UK Biobank were linked, some associations suggest when present, the Neandertal haplotype may infer improvement in fertility. Namely the observations imply more sisters, less haemorrhage in pregnancy and fewer miscarriages. | The relevance of this observation in IVF populations is of course unknown but since they are by definition, linked to infertility (both male and female), future studies may look at poor responders or recurrent miscarriage sub groups for whether the NPR is under represented. | Nevertheless, it illustrates how subtle genetic variations may have unseen impacts on the chance of success and that in the future, clinics may wish to fine tune treatment based upon a couple’s genetic profile and the V660L polymorphism may be one such candidate. | The NPR has been studied previously by others who noted that the primary variation was a valine to leucine substitution on the PR found on chromosome 11 (Copies of the PR is found in many chromosomes of the human genome of which there are two major and one minor types -A,B and C). Several other polymorphic changes have also been reported and suggest from the limited ancient fragments of DNA that the haplotype was heterogenous in the native populations. They are not found in African and North Asian populations while in European populations, Native Americans and some Asian populations, their frequency is cited as between 2 and 22%. In ancient populations it is thought to be homozygous present in about one third of individuals indicating there was some selective advantage. The authors noted that among ancient genomes, the haplotype V660L became progressively more common in western Eurasia. | The authors used the haplotype V660L to search the UK Biobank for links between it and standardized inpatient diagnosis associated with topics related to pregnancy and reported a negative association with bleeding and miscarriage. They further used the number of full siblings as a proxy for fertility and linked the number of sisters to the frequency of the haplotype. They then accessed data from the Genotype-Tissue Expression project to link the presence of the polymorphism to higher mRNA expression of the progesterone receptor. Taken together, the authors argued that the V660L Neandertal haplotype may infer some advantage on fertility and maintenance of pregnancy even though other work (DOI: 10.1016/j.ajhg.2018.05.009, 2018 ) have linked it to preterm delivery. The authors thought that may perversely been to sustain some pregnancies at risk of failure. They further argued that there did not appear to be any adverse events of carrying this haplotype. | So back to IVF and the role of progesterone. Many studies on many approaches have linked progesterone’s critical role in the transformation of the receptive uterine environment and the establishment of pregnancy. This and other papers detail the potential of individual genotypes to influence the actions of progesterone and possibly measuring serum concentrations do not fully identify function. As said above, in the future, as part of a client’s workup for IVF, a fully genetic profile may allow clinicians to more individualize and risk. | It is also kind of quirky the fertility problems faced by modern humans may also have been faced by ancient humans. When early humans mixed with ancient people, the introgression (the transfer of genetic information between species) included genes were thought to infer some immunological advantage but here we see possible selective reproductive advantage as well. So, if clients ask about Neanderthal genes, you can say it is a good thing!
