IVFD Increased risk of preterm birth in singleton pregnancies after blastocyst versus Day 3 embryo transfer: Canadian ART Register (CARTR) analysis
Preterm birth rate <37 weeks (unadjusted by potential confounding factors) was higher with Day 5/6 versus Day 3 transfers (17.2 versus 14.1%, P < 0.001). Using logistic regression analysis to adjust for confounding factors, preterm birth rate <37 weeks was the only outcome significantly increased after Day 5/6 compared with Day 3 transfer (odds ratio 1.32, 95% confidence interval 1.17–1.49).

IVFD Extended culture and the risk of preterm delivery in singletons: confounding by indication?
We read with interest the recent article (Dar et al., 2013) suggesting that singletons born after extended culture may have a higher risk of preterm delivery than those born after Day 3 transfer. While the importance of optimizing embryo culture and improving obstetrical outcomes after in vitro fertilization should not be underestimated, we believe this study suffers from a serious methodological flaw, namely ‘confounding by indication’.

IVFD Reply: Extended culture and the risk of preterm delivery in singletons: confounding by indication?
We read the above letter with interest and thank Drs Forman, Werner and Scott for their comments. We naturally agree that the goal of assisted reproduction techniques is a healthy baby, born at term, and that elective single embryo transfer (eSET) is the best way to avoid multiple pregnancy and its associated risk of preterm delivery and other complications.

IVFD Tetraploidy in hydatidiform moles
Among 20 HMs, 3 showed the genotype PPPM: one with the sex chromosomes XXYY and two with XXXY, indicating that they originated in tetraploid zygotes. In 14 androgenetic, one likely androgenetic and two mosaics, the tetraploid cells likely developed by endoreduplications of diploid cells. One case did not fulfil the histopathological criteria for HM.

IVFD Genotoxicity assessment of mouse oocytes by comet assay before vitrification and after warming with three vitrification protocols
Vitrification protocols containing PrOH induced significant DNA damage on mouse oocytes, both before cooling and after warming. Therefore, for the moment, we prefer vitrification techniques without PrOH while we await more studies on PrOH toxicity and long-term evaluation.

IVFD Bed rest after embryo transfer negatively affects in vitro fertilization: a randomized controlled clinical trial
The statistically significant higher LBI rate shown in our NR group confirms that 10 minutes of bed rest immediately after ET has no positive effect and in fact can be negative for the outcome of IVF with OD. The anatomical/physiological or psychological reasons for this should be explored in future research.

IVFD Cleavage-stage biopsy significantly impairs human embryonic implantation potential while blastocyst biopsy does not: a randomized and paired clinical
Blastomere biopsy on day 3 of development resulted in a significant reduction in sustained implantation. Only 30% of biopsied embryos had sustained implantation and ultimately developed into live-born infants versus 50% of unbiopsied controls, a relative reduction of 39%. In contrast, sustained implantation rates were equivalent (51% vs. 54%) for biopsied and control blastocysts.

This meta-analysis favors the use of ICSI to increase fertilization rates and decrease the risk of TFF in couples with well-defined unexplained infertility. Further studies are needed to determine the impact on clinical pregnancy and live birth rate.

Men with azoospermia have an increased risk of subsequently developing cancer, suggesting a possible common etiology between azoospermia and cancer development. Additional follow-up of azoospermic men after reproductive efforts end may be warranted.

Men who are diagnosed as azoospermic - infertile because of an absence of sperm in their ejaculate - are more prone to developing cancer than the general population, a study led by a Stanford University School of Medicine urologist has found. And a diagnosis of azoospermia before age 30 carries an eight-fold cancer risk, the study says.
