The birth of many healthy children has enhanced provider and patient confidence in the safety of IVF. But does applying IVF to wider forms of infertility result in overtreatment of couples who had a reasonable chance of conceiving naturally? Is it equally effective in these conditions? And, as more is understood about the adverse health outcomes in IVF children can the risks of IVF be justified for these more liberal applications?

Although on its own the newly proposed very low normal value may not provide the strong
predictive value for a males’ fertility potential, as originally reported for sperm morphology evaluated according to strict
criteria, a good predictive value can still be obtained if the holistic, strict approach for sperm morphology evaluation is
followed together with additional sperm morphology parameters now available, because certain morphology pa
Are the risks of extending IVF outweighing the benefits?
As the use of IVF expands to a wider range of fertility problems, experts warn that the risks could outweigh the benefits. IVF was developed for women with fallopian tube disorders and severe male infertility, but in recent years it has been applied to wider conditions, including unexplained infertility, write Dr Esme Kamphuis and colleagues on bmj.com today.
Divorce or end of cohabitation among Danish women evaluated for fertility problems
After up to 12 years of follow up, nearly 27% of the women were no longer living with the person with whom they had lived at the time of the fertility evaluation. Women who did not have a child after the evaluation had significantly higher odds ratios for ending a relationship up to 12 years after the evaluation (with odds ratios up to 3.13, 95% CI 2.88–3.41) than women who had a child, regardless of their parity before the evaluation.
Obesity reduces uterine receptivity: clinical experience from 9,587 first cycles of ovum donation with normal weight donors
Result(s): In vitro fertilization laboratory parameters did not differ according to BMI. However, implantation, pregnancy, clinical pregnancy, twin pregnancy, and live-birth rates were significantly reduced as BMI increased. In the lean, normoweight, overweight, and obese groups, the implantation rate was 40.4%, 39.9%, 38.5%, and 30.9%, clinical pregnancy rate was 56.9%, 55.9%, 54.3%, and 45.3%, and live-birth rate was 38.6%, 37.9%, 34.9%, and 27
Additive effect of factors related to assisted conception on the reduction of maternal serum pregnancy-associated plasma protein A concentrations and
Result(s):NT is unaffected by the mode of conception. Singleton pregnancies achieved by IVF and ICSI with nondonor oocytes have reduced maternal serum PAPP-A and increased FPR, which are significant only in ICSI cycles. Pregnancies from frozen embryos with hormone therapy also show decreased PAPP-A but without affecting the FPR. Elevated maternal serum fβ::hCG levels in oocyte donation do not influence the FPR. Conclusion(s):Among assisted co
Is it acceptable to destroy or include human embryos before day 5 in research programmes?
These pregnancies resulted in five miscarriages and five healthy live births at full term. PQE may achieve their development to the blastocyst stage, be frozen–thawed and harbour reasonable implantation potential. These results, thereby, raise an ethical issue regarding the fate reserved to PQE.[Poor Quality embryos a must read article. ed]
Influence of temperature and sperm preparation on the quality of spermatozoa
However, spermatozoa prepared by swim-up showed better survival after incubation compared with DGC-prepared spermatozoa, especially when incubated at 35°C. In conclusion, this study indicates a significantly better and longer preservation of sperm quality when incubation is performed at RT. These findings may convince laboratories to change the routinely used sperm storage conditions in order to maximize the quality of the prepared sperm sample.
Maternal characteristics and pregnancy outcomes after assisted reproductive technology by infertility diagnosis: ovulatory dysfunction versus tubal ob
Conclusion(s)Women with OD/PCOS who underwent ART have different characteristics and health issues (higher prevalence of diabetes) and infant outcomes (lower Apgar score) compared with women with TO. [Comment: Infertility should flag other health investigations or limit stimulation?]
In vitro effects of coital lubricants and synthetic and natural oils on sperm motility
Conclusion(s): Sesame oil and synthetic coital lubricants impaired sperm motility and may hamper fertility. Pre-Seed and canola, mustard, and baby oils showed no deleterious effect and may be considered sperm-friendly coital lubricants. Mustard oil exposure resulted in hyperactivation of sperm and needs to be studied further.
Oocyte number as a predictor for ovarian hyperstimulation syndrome and live birth: an analysis of 256,381 in vitro fertilization cycles
Conclusion(s): Retrieval of >15 oocytes significantly increases OHSS risk without improving LB rate in fresh autologous IVF cycles. In general, less aggressive stimulation protocols should be considered, especially in high-responders, to optimize outcomes. [Comment:One of many studies linking OHSS with 15+ oocytes- Seems to be the upper clinical limit for stimulation]
Rescue intracytoplasmic sperm injection: a systematic review
Result(s): Thirty-eight studies including 1,863 patients were included. The pooled pregnancy rate was 14.4%: 194 babies were delivered. - Conclusion(s): Rescue ICSI can result in the delivery of a healthy newborn, although the pregnancy rates are low. The clinical evidence did not indicate an elevated rate of malformations, although the data are limited and incomplete.
Effect of sugar molecules on the cryopreservation of mouse spermatogonial stem cells
Result(s): Supplementation of freezing medium with 200 mM disaccharide is an effective method for cryopreservation of SSCs. Trehalose is the most effective cryoprotectant among all the sugars tested and only lactose was comparable to trehalose. Our proliferation and transplantation data show that serum-free freezing can be achieved in freezing medium supplemented with 200 mM trehalose, 10% knockout serum replacement, and 10% dimethyl sulfoxide (D
IVF And Divorce: Couples Three Times More Likely To Break Up After Failed Fertility Treatment
The question as to whether having children puts a strain on relationships has angered some and made others question their relationships. In 2011, a study conducted by OnePlusOne, a UK charity that helps couples with relationship issues, found that the toughest period in a marriage occurred during transition to parenthood, with couples `failing to see the problems` until it was too late, the Daily Mail reported.
Fertility doctors have raised concerns that Australian couples with unexplained fertility issues are rushing into IVF treatment when they could go on to conceive naturally. An article published in the British Medical Journal says IVF (in vitro fertilisation) offered men with serve infertility and women with blocked fallopian tubes the chance to have a child but the technology is now being overused.
Science of baby-making still a mystery for many women
A new study provides insight into how much women of reproductive age in the United States know about reproductive health. The study found that about 50 percent of reproductive-age women had never discussed their reproductive health with a medical provider and about 30 percent visited their reproductive health provider less than once a year or never.
Low vitamin D levels during pregnancy may increase risk of severe preeclampsia
Women who are deficient in vitamin D in the first 26 weeks of their pregnancy may be at risk of developing severe preeclampsia, a potentially life-threatening disorder diagnosed by an increase in blood pressure and protein in the urine, according to research.
Couple who had son with cystic fibrosis to sue doctors, fertility practice
A COUPLE who had a son with cystic fibrosis, despite having genetic tests to reduce that risk, has filed a $1.8 million lawsuit against two doctors and a Brisbane fertility practice. The male had cystic fibrosis and he and his partner wanted to avoid giving birth to a child with the same condition, after undergoing in-vitro fertilisation, their claim says.
