Welcome to Patient/Client Survey Facility.

This facility allows to you complete a client survey online for your clinic or company. The survey is a series of questions with 5 reply options ( 1=Poor, 3 = Average to 5 very good). It will take only 1-2 minutes to complete the survey. You clinic will be able to see your submissions at any time. They will have provided you with a Code and a password to select the survey and possibly a unique identifier to use that may ensure that your submission is a valid and can be used. The opportunity exists to provide your name but this is optional.
