Welcome to FertAid Surveys - part of the Accreditation tools portfolio.
This sections allows you to create a survey, create, define and link questions to the survey, add and edit responses and create reports for your management and accreditation process. Adding questions from previous surveys allows you to compare your performance with previous suveys. You nee to be registered with QAPonline to access these facilities. Each survey is linked to your QAP group. Each survey has a unique logon and password so you can use the external data entry capabilities independently of the QAP group- e.g. your pateints can add the results directly on-line.
