IVFD Does the meiotic spindle really predicts embryo implantation and live birth rates? An update
Despite the different methodology used in all the chosen studies, presence of the MS in oocytes was found to be positively associated with embryo implantation. Moreover, high retardance values, as well as strict criteria of normality in the MS structure, are significantly related to higher embryo implantation numbers and live birth rates.

Conclusion: Meiotic spindle imaging may be a valuable tool for prediction of oocyte quality, and retardance values of meiotic spindles, together with classical morphological classification, can be useful to select embryos with a higher implantation potential.

Conclusion: Our study showed that the microfluidic technique does not change fertilization, CPR, and LBR during IVF treatment for couples with unexplained infertility. Despite the fact that the total number of grade 1 embryos after ICSI treatment and the surplus number of grade 1 embryos after embryo transfer were higher in the microfluidic technique group, the study was not powered to detect this difference.

Does the use of a Sperm Selection tool for ICSI improve embryo quality and pregnancy rates?

In this prospective randomised trial, the use of a microchip (Fertile Chip) over swim-up methodology for preparing sperm for ICSI, 61 couples used sperm prepared by the above microchip tool and another 61 couples use sperm prepared by swim-up protocols. The authors were not able to show any difference in the pregnancy rate between the two methods but did show even in this relatively small study group), significantly more good quality embryos and more surplus usable embryos.|The tool has been shown to deliver a sperm sample of low DNA fragmentation that some may argue is similar to the sperm that binds to the zona and effects normal (ie IVF insemination) fertilisation. Sperm with low DNA fragmentation have been shown to be associated with lower miscarriage rates. As such, they represent a pool of sperm preferred for IVF/ICSI.| In effect, sperm with low DNA fragmentation also have good motility and normal morphology as does sperm from swim-up methodology. So it is not surprising that there was no difference in the pregnancy rate. What was interesting was they were able to show better embryo quality.| Using a defined process to restrict the sperm to a defined population using the microchip has some important advantages not the least being a form of standardisation for sperm used in IVFICSI. It is a problematic aspect of current IVF technology especially for ICSI that the sperm selected by the embryologist for the insemination of the oocytes in each cohort is largely unregulated and at the mercy of the scientists own preferences and skills. The clinic to a large extent has little control over what sperm is injected and therefore on many aspects including embryo quality. Using a microchip at least defines the population used for sperm selection to a more homogeneous one. Even though there is a cost to using the chip, one may (and should) argue that the cost is worthwhile if it ensures all the sperm injected at ICSI has the lowest risk of generating an embryo that may miscarriage. Given the choice, it is almost certain the clinics clients would request it and if they miscarry could ask why!|
