Decrease in American birth rates makes it imperative for the United States to implement state mandated fertility coverage
Given the high cost of fertility treatment, this option is not valid for families with low income, and those who can afford it usually choose to have multiple embryo transfers, which has led to an increase in multiple birth rates and increase in the cost of perinatal care. Due to the expected increase in infertility and decrease in the national live birth rate, the United States should set a plan to fund infertility treatment and lead a policy fo
Perifollicular vascularity in poor ovarian responders in In vitro fertilization cycles
Results: A total 49 patients were recruited for the study.10 patients were allocated in Group A, 26 patients in Group B and 13 in Group C. Both groups were comparable in terms of age, period of infertility, follicle-stimulating hormone, luteinizing hormone, and gonadotrophin requirement. The number of metaphase II oocytes and good quality oocytes was significantly higher in Group C. Implantation rate, clinical pregnancy rates were similar.
Evaluation of the morphokinetic parameters and development of pre‐:implantation embryos obtained by testicular, epididymal and ejaculate spermato
The morphokinetic parameters of time intervals between the second polar body (PB2) extrusion, pronuclei formation and disappearance and cleavage divisions showed significant variations in TESE, MESA and control groups. Furthermore, the pregnancy rates (positive beta‐::hCG) were shown to be similar in both TESE and the control group (55% in each group), whereas for the MESA group, this rate was significantly lower (39%, p = 0.049). Revisiting the impact of ejaculatory abstinence on semen quality and intracytoplasmic sperm injection outcomes
Conclusions: Ejaculatory abstinence periods of >4 days have a detrimental effect on sperm DNA and intracytoplasmic sperm injection outcomes. One day of ejaculatory abstinence significantly improves implantation rate and tends to increase pregnancy rate, compared to 2, 3 and 4 days of ejaculatory abstinence.
The Sertoli cell: one hundred fifty years of beauty and plasticity
It has been one and a half centuries since Enrico Sertoli published the seminal discovery of the testicular ‘nurse cell’, not only a key cell in the testis, but indeed one of the most amazing cells in the vertebrate body. In this review, we begin by examining the three phases of morphological research that have occurred in the study of Sertoli cells, because microscopic anatomy was essentially the only scientific discipline available for about th
Effects of the microfluidic chip technique in sperm selection for intracytoplasmic sperm injection for unexplained infertility: a prospective, randomi
Conclusion: Our study showed that the microfluidic technique does not change fertilization, CPR, and LBR during IVF treatment for couples with unexplained infertility. Despite the fact that the total number of grade 1 embryos after ICSI treatment and the surplus number of grade 1 embryos after embryo transfer were higher in the microfluidic technique group, the study was not powered to detect this difference.
The Impact of the Law in Helping or Hindering Fertility Preservation for Children with Cancer Facing Gonadotoxic Therapies.
Children diagnosed with cancer who require treatment with chemotherapy and/or radiation therapy have ever-increasing survival rates. However, as a result of such treatment they face the added, and significant, burden of infertility into their futures. Options for fertility preservation and future reproduction for such children do exist, but some such options continue to be considered experimental.

Title
Should IVF be used as a tool to manage population growth?
Summary
In this article in the Journal of Medical Economics, the authors argue that with increasing male infertiity and falling birthrates, most countries should consider using IVF as a management tool
Comment
The authors develop an argument that with falling birthrates mainly due to the delaying the age of childbearing for acquiring higher education and career and with falling semen quality (increasing in male infertility), IVF may be a valuable tool for countries to consider to stabilise their birth rates.|While initially multiple pregnancy rates were a reason to not consider IVF, recent trends in the use of SET means the cost of IVF to the state is lower. The authors also argue that for many, IVF remains too expensive and when used, sought to maximise multiple pregnancy rates. IF the government addressed the issue to cost of access, then IVF can be used across all parts of society to contribute to birth rates.|This is the first time that the issue of increasing male infertility and falling birthrates have been linked together in the social context of maintaining birthrates as a social policy. One may expect to see more of this arguement in the future.|
Title
Does the use of a Sperm Selection tool for ICSI improve embryo quality and pregnancy rates?
Summary
Yes it improves embryo quality and No, it does not improve pregnancy rates
Comment
In this prospective randomised trial, the use of a microchip (Fertile Chip) over swim-up methodology for preparing sperm for ICSI, 61 couples used sperm prepared by the above microchip tool and another 61 couples use sperm prepared by swim-up protocols. The authors were not able to show any difference in the pregnancy rate between the two methods but did show even in this relatively small study group), significantly more good quality embryos and more surplus usable embryos.|The tool has been shown to deliver a sperm sample of low DNA fragmentation that some may argue is similar to the sperm that binds to the zona and effects normal (ie IVF insemination) fertilisation. Sperm with low DNA fragmentation have been shown to be associated with lower miscarriage rates. As such, they represent a pool of sperm preferred for IVF/ICSI.| In effect, sperm with low DNA fragmentation also have good motility and normal morphology as does sperm from swim-up methodology. So it is not surprising that there was no difference in the pregnancy rate. What was interesting was they were able to show better embryo quality.| Using a defined process to restrict the sperm to a defined population using the microchip has some important advantages not the least being a form of standardisation for sperm used in IVFICSI. It is a problematic aspect of current IVF technology especially for ICSI that the sperm selected by the embryologist for the insemination of the oocytes in each cohort is largely unregulated and at the mercy of the scientists own preferences and skills. The clinic to a large extent has little control over what sperm is injected and therefore on many aspects including embryo quality. Using a microchip at least defines the population used for sperm selection to a more homogeneous one. Even though there is a cost to using the chip, one may (and should) argue that the cost is worthwhile if it ensures all the sperm injected at ICSI has the lowest risk of generating an embryo that may miscarriage. Given the choice, it is almost certain the clinics clients would request it and if they miscarry could ask why!|
