on to www.fertaid.com and select "FertAid" to open
the home page for QAPonline. Since all QAPonline requires
to know who it is dealing with, the first procedure is to
register. |

| At
the FertAid home page you may either register if you have
done this previously or logon if you already have a logon
code and password. It is not recommended that you register
more than once since QAPonline checks for surname and email
address and may block subsequent logon/password combinations. |
